Session Summary
Provide a clear and concise summary of the main issues discussed by the clinician and the client in today’s session. Highlight key topics, any insights the client gained, and any emotions or themes that stood out.
Activities For You To Do
- List all the tasks, exercises, or reflections that the therapist has suggested and that the client has agreed to do before the next session.
- Be specific, and write them in a friendly, encouraging tone.
- These might include evidence-based self-help strategies, self-care activities, tasks that need to be completed (such as obtaining a new referral, visiting a different specialist), or anything else discussed.

- Practice the 5-minute Progressive Muscle Relaxation audio I’ve emailed you once each day, preferably before your morning commute.
- Draft bullet points for your upcoming presentation and review them with your supportive colleague Jamie before our next session.
- Try the “5-4-3-2-1” grounding technique when you notice your anxiety building, and make a quick note of how effective it was.
- Attend at least one team lunch this week, staying for a minimum of 20 minutes as we discussed.
